Please remember that upon your arrival at The Excelsior Springs Hospital,
you will be asked to verify your registration information, a copy of your
insurance card will be made and you will be asked to sign authorization
forms. If you have any questions about pre-registration, please call

Registration requirements include:
Procedure/Surgery Pre Certification
Insurance Card
Valid Picture ID - Used for security purposes and to protect against identity theft
Physician's Order
You may also be asked to provide co-payment or deposit
