Continuum of Care
What is Continuum of Care?
Continuum of Care means being able to meet all of your medical
needs in one building. We offer residents three levels of care to fit their medical and social
needs. They may begin their stay in our Independent Living Center and then feel
they need more medical assistance or oversight and can transfer to our Residential Care or
Convalescent Center. They continue to have many of the same staff and never leave
the medial complex. This helps make the change less disruptive to the individual. Because it is
hospital based, it provides residents the convenience of our outpatient services,
specialists, rehabilitations services, hospital, and emergency departments
without leaving the medical complex.
Convalescent Center
The Convalescent Center is a 60-bed hospital based unit. The unit is
small in size, which means staff members are able to understand the
uniqueness of every Resident, their interests, and personalities. We
encourage, the highest quality of life, maintain dignity and respect and
value family involvement in the lives of our Residents. Our activity
department offers daily activities and encourages Residents continued
involvement in the community by scheduling many out-of-facility activities.

Our patients enjoy surroundings that have been designed to provide quality of life for them, their family and friends. Residents can visit with guests in their rooms or in comfortably furnished lobbies. An activities room allows residents to pursue hobbies. A well-landscaped outdoor terrace with shelter house and decorative fountain provides a restful atmosphere.

Residential Care Center
As part of our Continuum of Care, our Residential Care facility offers a
protective environment for those elderly individuals who no longer are,
or do not feel safe in their homes. It is a 20-bed, home like environment
unit, which offers assistance with personal cares, medication management,
three meals daily, housekeeping daily and 24-7 nursing and physician
supervision. Large private, smaller private and semi-private rooms are
available with private baths along with access to our beautiful courtyard.
